VOTE BY MAIL (ABSENTEE BALLOT) INSTRUCTIONS



Are you an active duty member of the United States Armed Forces, spouse or dependent of those active duty members or an United States citizen residing overseas?

Any qualified Nassau County voter is entitled to a Vote by Mail ballot.  A Vote by Mail ballot may be requested in person at either of our offices, in writing, over the telephone or by completing our online VOTE BY MAIL BALLOT REQUEST FORM.

The voter requesting a Vote by Mail ballot must provide the following information:

·        Name

·        Residence address

·        Address where the ballot is to be mailed

·        Date of birth

·        Local telephone number

·        Out of town telephone number

·        Voter’s signature (written requests only)

 


If a member of the voter’s immediate family* is requesting the Vote by Mail ballot, the requestor must provide the voter’s information listed above, as well as the following:


·        Requestor’s name (if other than the voter)

·        Requestor’s relationship to the voter

·        Requestor’s address

·        Requestor’s driver’s license number (if available)

·        Requestor’s signature (written request only)


  *Immediate family is defined as: spouse, child, grandparent or sibling.



PLEASE REMEMBER: VOTE BY MAIL BALLOTS CANNOT BE FORWARDED.



A designee may pick up an absentee ballot for a voter on Election Day or five (5) days before Election Day.  Designee must provide:

 

·        Written authorization signed by the voter

·        Present picture identification

·        Sign an affidavit containing all required information for voter and designee


You may print and use the AUTHORIZATION / AFFIDAVIT FORM, which is provided for your convenience. 


PLEASE NOTE:Florida Statute 101.62(4)(b)4 limits pick up of Vote by Mail ballots by a designee to two (2) ballots per election, other than their own or those for immediate family.

 



Specific instructions are provided on the actual Vote by Mail ballot and the secrecy sleeve enclosed in the Vote by Mail kit mailed to you. It is imperative that you follow these instructions very carefully.  The voter must personally vote the ballot, unless assistance is required due to disability.

 

In order for your ballot to be counted, your Voter’s Certificate Envelope must contain your signature.  Florida Statutes require the County Canvassing Board to reject as illegal any Vote by Mail ballot that is not signed.

 


Your voted and signed Vote by Mail ballot must be received by the Office of the Supervisor of Elections by 7:00 p.m. on Election Day in order to be counted.
